Owner report 10610639 · 2014-07-15 (July 15, 2014)
THIS IS THE SECOND OCCURRENCE OF THE SAME ISSUE, BUT WHILE DRIVING THE VEHICLE AT HIGHWAY SPEEDS, ALL DASH LIGHTS ILLUMINATED (CHECK ENGINE, TURN SIGNALS, TEMP WARNINGS, ETC) SIMULTANEOUSLY AND THE INFORMATION SCREEN DISPLAYED AN ALERT "ENGINE INSPECTION REQUIRED". THE INCIDENT ALSO RESULTED IN A LOSS OF POWER STEERING/HEAVIER-THAN-NORMAL STEERING EFFORT AND BRAKE WARNING INDICATIONS, ON THE FIRST INSTANCE (JULY 10, 2014), MAZDA ROADSIDE ASSISTANCE WAS CONTACTED AND THE VEHICLE WAS TOWED TO THE DEALER WITH NO FAULT/CAUSE FOUND. ON THE SECOND IN…

DRIVER SIDE VIEW MIRROR WILL VIBRATE WHILE DRIVING AND HARD TO SEE THE OBJECTS PROPERLY IN THE MIRROR. THEY HAD THE SAME ISSUE ON 2013-2014 VEHICLES. WHEN I CONTACT MAZDA DEALERS, THEY SAY THERE IS NO FIX FOR THAT AND GETTING AWAY WITHOUT TRYING TO FIX IT. CLEARLY IT IS A SAFETY ISSUE. *TR

I BOUGHT MY MAZDA CX5 NEW A 2015 MODEL YEAR AND EXPERIENCED A THUMPING NOISE WHEN WE SLIGHTLY PUT REAR WINDOWS DOWN. IT WASN'T JUST A NOISE THOUGH, THE PRESSURE IN THE CAR CHANGED AND CAUSED BOTH MY HUSBAND AND MYSELF HEADACHES. I HAVE REPORTED IT TO THE SERVICE MANAGER AT THE DEALER WE BOUGHT IT FROM AND THEY SAID THE MANUFACTURER IS AWARE OF IT AND THEY HAVE A FIX FOR IT BUT YOU HAVE TO PURCHASE IT FROM THEM TO GET RID OF THE NOISE. I HAVE READ OF THE SAME COMPLAINT REGARDING THE 2013 AND 2014 YEARS. IF I HAD KNOWN ABOUT THIS PROBLEM AND…

Owner report 10641377 · 2014-10-04 (October 4, 2014)
VEHICLE PARKED. GOT INTO VEHICLE AND NOTICED LARGE CRACK IN WINDSHIELD. THE CRACK WAS NOT THERE 2 HOURS EARLIER WHEN I GOT OUT OF IT. THE CRACK STARTS AT TOP CENTER IN THE FRONT WINDSHIELD AND RUNS DOWN TO THE REAR VIEW MIRROR AND FORMS A 'S' SHAPE AND STOPS BENEATH THE MIRROR. IT IS ABOUT 18 INCHES LONG. THE REAR VIEW MIRROR IS ATTACHED TO A PANEL THAT HAS RAIN SENSOR AND SMART STOP TECH PACKAGE ATTACHED TO CENTER OF WINDSHIELD RIGHT UNDER WHERE THE CRACK APPEARS. THIS IS A SAFETY CONCERN AND EXPENSIVE TO REPLACE. I AM SEEING EVIDENCE OF…

Owner report 10641575 · 2014-10-06 (October 6, 2014)
FIRST INCIDENT, CRACKED WINDSHIELD, APPEARED OVER NIGHT. STARTED OUT ABOUT 8 INCHES LONG AND GREW TO ABOUT 18 INCHES. I COULDN'T GET A REPLACEMENT FOR 2 WEEKS -THERE WAS NO INVENTORY. JUST HAPPENED AGAIN, NOTICED ON OCTOBER 5 A CRACK THAT IS A FOOT LONG. AGAIN, HAPPENED WHILE THE VEHICLE WAS SITTING OVER NIGHT IN MY GARAGE. VEHICLE NOW HAS ONLY 3200 MILES. 2 CRACKS IN A SHORT PERIOD OF TIME AND NO INVENTORY OF REPLACEMENT PARTS MEANS I HAVE TO DRIVE WITH POOR VISIBILITY. I'M ALSO CONCERNED ABOUT HOW THE WINDSHIELDS WOULD HOLD UP IF T…

Owner report 10648268 · 2014-10-16 (October 16, 2014)
I WAS PARKING MY CAR IN THE GARAGE LAST NIGHT, WHEN I NOTICED A FRACTURE LINE PROPAGATING FROM THE BASE OF THE WINDSHIELD (ABOUT 6 INCHES LONG). IN MY ENTIRE HISTORY OF DRIVING I HAVE NEVER SEEN THIS WITH A VEHICLE. THERE WAS NO STONE CHIP OR INDENTATION. JUST A FRACTURE LINE. BY THE NEXT DAY WHEN I GOT INTO MY CAR, THE FRACTURE LINE HAD SPREAD ACROSS THE WINDSHIELD. MAZDA CLAIMS HIGH SAFETY RATING, BUT THIS IS A MANUFACTURE ERROR AND POOR/THIN GLASS QUALITY. THE DEALERSHIP WAS TRYING TO ARGUE AND NOT TAKE RESPONSIBILITY. THIS IS A MAJOR SA…

Owner report 10654312 · 2014-11-12 (November 12, 2014)
OUR VEHICLE IS EQUIPPED WITH THE "SMART CITY BRAKING" FEATURE THAT AUTOMATICALLY PREVENTS COLLISIONS AT LOW SPEED. THIS SYSTEM TRIGGERS THE BRAKES AND BRINGS THE VEHICLE TO A STOP WHEN AN OBJECT IS DETECTED IN FRONT OF THE VEHICLE. THIS SYSTEM HAS BEEN TRIGGERED BY STEAM CLOUDS COMING OUT OF SEWER GRATES AND HAS BROUGHT THE CAR TO A COMPLETE AND VERY UNEXPECTED STOP SEVERAL TIMES. NO REAL "OBJECT" WAS BEHIND THE STEAM CLOUD / YET BECAUSE OF THE WAY THE SYSTEM IS IMPLEMENTED IT DETECTED AN OBJECT AND ACTIVATED THE BRAKES. THIS UNEXPECTED B…

HAVE HAD THE CAR FOR APPROXIMATELY A MONTH AND HAVE GOTTEN 2 LARGE CHIPS IN THE WINDSHIELD FROM UNHEARD AND UNSEEN OBJECTS. UPON ATTEMPT TO REPAIR BY GLASS REPAIR COMPANY THE WINDSHIELD PROCEEDED TO CRACK OVER 15 INCHES. I AM REPLACING THE WINDSHIELD WITH NON OEM GLASS AS THERE APPEARS TO BE AN ISSUE WITH THE OEM GLASS CONSIDERING THE MULTIPLE REPORTS BY OTHER OWNERS. *JS

Recall history
3 distinct campaigns affect this selection. Announcement dates below are different from vehicle model years. A campaign may cover multiple years.
16V064000 · Fuel System, Gasoline:Storage:Tank Assembly:Filler Pipe And Cap
Reported: 2016-02-03 (February 3, 2016)
Mazda North American Operations (Mazda) is recalling certain model year 2014-2016 Mazda CX-5 vehicles manufactured June 27, 2013, to February 2, 2016. The affected vehicles have a fuel filler pipe that may rupture in the event of a crash. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 301, "Fuel System Integrity."
Consequence: A rupture of the fuel filler pipe may result in a fuel leak, increasing the risk of a fire.
Remedy: Mazda will notify owners, and dealers will alter the attachment between the fuel filler pipe and the left rear side of the bracket of the fuel filler pipe, free of charge. The recall began on February 25, 2016. Owners may contact Mazda customer service at 1-800-222-5500, option 6. Mazda's number for this recall is 9216B.

Reported: 2016-04-11 (April 11, 2016)
Mazda North American Operations (Mazda) is recalling certain model year 2016 CX-3 vehicles manufactured June 1, 2015, to December 2, 2015, 2013-2014 Mazda2 vehicles manufactured July 3, 2013, to August 8, 2014, 2014-2016 CX-5 vehicles manufactured January 19, 2013, to December 2, 2015, and 2013-2015 CX-9 vehicles manufactured December 6, 2012, to July 31, 2015. The lower mounting nuts and bolts that attach the front strut assembly to the steering knuckle may loosen, allowing the steering knuckle to separate.
Consequence: If the steering knuckle separates from the strut assembly, it can cause a loss of steering control, increasing the risk of a crash.
Remedy: Mazda will notify owners, and dealers will inspect the lower mounting nuts and bolts that secure the front strut assembly to the steering knuckle. If the bolts are loose, they will be replaced and then correctly tightened, free of charge. The recall is expected to begin May 20, 2016. Owners may contact Mazda customer service at 1-800-222-5500. Mazda's number for this recall is 9316D.
