Incident 1: Car suddenly applied brakes while traveling at 60 MPH on the fast lane. There was no vehicle in the front for at least 50 ft. ahead. Incident2: Car suddenly applied brakes while traveling at about 40 MPH. Again no vehicle in the front for at least 100 ft. in the front. Phantom Braking when using Adaptive Cruise control. The vehicle brakes unexpectedly when adaptive cruise control is enabled. Typically happens when there is no leading vehicle in my lane, but there are vehicles ahead of me in other lanes. Happens consistently, almost every time the adaptive cruise is enabled. The system also applies brakes in situations when cruise control is not enabled, and there is no leading vehicle in front of me. Issue can surprise drivers behind me and lead to a collison where I am hit from behind.

@ 96,970 miles, (2021 Honda Accord Hybrid) when getting on the freeway and accelerating to catch up with traffic, the car began sputtering, the check engine light flashed, the accelerator stopped working and I had to get it to the shoulder of the freeway immediately. The car sputtered and shook. I turned the car off, let it sit. I tried re-starting it and it started fine, no check engine light. I drove it slow off the freeway and to my home close by. The contact owns a 2021 Honda Accord. The contact stated while driving at approximately 75 MPH, the vehicle stalled. The contact waited for several minutes for the vehicle to return to normal operation. The contact stated that several unknown warning lights were illuminated. The vehicle was taken to a dealer where it was diagnosed that the fuel injectors needed to be replaced. The vehicle was repaired. The manufacturer was not notified of the failure. The failure mileage was 71,274.

Most recent incidents on Friday 08/07 car lost power while driving on freeway unable to accelerate, all engine/warning lights came on was thankfully able to pullover and shut off vehicle. This is not first incident this also happened about 5x in 2025 and had car inspected and serviced by Honda dealership each time being told they didn’t know what the issue is because the engine light wasn’t on at time of appointments. Upon pulling into a parking space, with the steering wheel almost at full lock to the right, when I braked, the car suddenly accelerated out of control and hit another car. Airbags did not deploy but there was damage to both vehicles, no injuries. Vehicle has been taken to a collision repair shop to await repair. It is available for inspection. Has not been looked at to date.
