Owner report 11536313 · 2023-08-03 (August 3, 2023)

Driver was parking the car in a parking lot at a very slow speed. The car suddenly shot forward at a very high rate of speed, requiring evasive steering from trees and a fence, until the car crashed head-on into another vehicle. The driver is very certain the brakes only were applied during the entire time the car shot forward. Airbags were not deployed. Bystanders were within six feet of the car at one point during the acceleration, prior to the crash. There were no injuries. Earlier, on the day of the crash, the car had a sudden, stro…

Owner report 11552598 · 2023-10-30 (October 30, 2023)

Distracting connected services spam. When the car is turned on, the center console displays a "safe driver" warning, then after a number of seconds, a message instructing driver to sign up for connected services for navigation appears and won't display a default screen or last screen displayed until driver selects another display option. I was never on the navigation menu, nor do intend to ever use the navigation menu, and yet the solicitation appears. This is not only annoying, but distracts the driver from other information that the driver i…

Owner report 11622380 · 2024-10-28 (October 28, 2024)

A Toyota advertisement for an optional subscription service, called Drive connect, takes over the display randomly, including when I am using the display for directions with GPS. There have been many instances where I have been using the display for directions, when this banner takes over the screen, and I have to abruptly reach over and turn it off so that I can continue following the directions. I have missed turnoffs because of this. And I do not think that it is safe to have to reach over suddenly to switch the display back to GPS again. I …

Owner report 11632312 · 2024-12-25 (December 25, 2024)

I was pulling slowly into my driveway and suddenly the car began accelerating on it's own. I braked and it would not stop. I ended up running into my electronic gate and hitting the side of my neighbor's house. Safety was put at risk because I could have hit a person or moving car had this not happened in my driveway. I could have caused more substantial damage to my neighbor's house. I will report this to the dealership and have it inspected. There were no warning lamps or messages or warning symptoms.

Owner report 11632869 · 2024-12-28 (December 28, 2024)

I slowly turned right into a parking space, braked to stop but the car didn't stop and instead lurched, jumping forward over the parking curb and hitting a small wall. My foot was on the brake pedal the entire time. It happened at 9:00am on a clear day. I was alone in the car and uninjured. Damage to the car was extensive and estimated at nearly $40,000. As a result, car was totaled by the insurance company.

Recall history

2 distinct campaigns affect this selection. Announcement dates below are different from vehicle model years. A campaign may cover multiple years.

25V595000 · Electrical System: Instrument Cluster/Panel

Reported: 2025-09-11 (September 11, 2025)

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2023-2024 Toyota Venza, 2023-2025 RAV4 Prime, RAV4, Highlander, GR Corolla, Crown, 2024-2025 Lexus TX, LS, Toyota Tacoma, Grand Highlander, and 2025 Lexus RX, Toyota Crown Signia, Camry, RAV 4 Plug-in Hybrid (PHEV), and 4 Runner vehicles. Due to an error in the instrument panel software at vehicle startup, the instrument panel may fail to display vehicle speed, brake system, and tire pressure warning lights.

Consequence: An instrument panel display that does not show critical information can increase the risk of a crash or injury.

Remedy: Dealers will update the instrument panel software over-the-air (OTA) for non-PHEV vehicles, free of charge. For PHEV vehicles, dealers will inspect the instrument panel assembly, and either replace it, or update the software, free of charge. Owner letters were mailed December 5, 2025. Additional letters will be sent, anticipated in May 2026. This is a phased recall. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB08 and 25TA08. Lexus' numbers for this recall are 25LB05 and 25LA05.

Reported: 2025-10-30 (October 30, 2025)

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2026 Toyota, Lexus, and Subaru Solterra vehicles equipped with a Panoramic View Monitor (PVM) system. Please see the recall report for a complete list of models. A software error may cause the rearview camera to freeze or display a blank screen when the vehicle is in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."

Consequence: A rearview camera that fails to display an image can reduce the driver's view behind the vehicle, increasing the risk of a crash.

Remedy: Dealers will update the parking assist software, free of charge. Owner letters were mailed January 2, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB13 and 25LB06. Subaru's number for this recall is WRE25.
