TL* THE CONTACT OWNS A 2008 FORD F-650. THE CONTACT WAS DRIVING 35 MPH WHEN HE APPLIED THE BRAKES AND THE PEDAL WAS ABNORMALLY DEPRESSED INTO THE FLOORBOARD. THE VEHICLE DID NOT RESPOND AND THE CONTACT HAD TO SHIFT INTO NEUTRAL IN ORDER TO STOP THE VEHICLE. THE VEHICLE WAS TAKEN TO THE DEALER WHO REPLACED THE MASTER CYLINDER AND CLUTCH SLADE BUT THE BRAKES CONTINUED TO FAIL. THE CONTACT STATED THAT THE CLUTCH SLADE CYLINDER AND MASTER CYLINDER FAILED AND WAS REPAIRED FOUR TIMES. THE MANUFACTURER WAS NOTIFIED OF THE FAILURE WHO PROVIDED A PARTIA…

23-YEAR-OLD SUBJECT WAS A PASSENGER IN A LIMOUSINE/BUS WHICH HAD BEEN BUILT OUT BY LIMOS BY TIFFANY, INC. SHE WAS LOCATED NEAR THE EMERGENCY EXIT WINDOW AREA. EMERGENCY LATCH WAS IN CLOSED POSITION AND WINDOW APPEARED CLOSED; HOWEVER, WHEN DRIVER NEGOTIATED A LEFT CURVE, SUBJECT MADE CONTACT WITH THE WINDOW AND IT OPENED, CAUSING HER TO FALL OUT ONTO THE ROADWAY AND SUSTAIN SERIOUS INJURIES. POST-ACCIDENT TESTING DEMONSTRATED LOCKING HANDLE COULD BE IN THE CLOSED POSITION YET THE WINDOW COULD BE PUSHED OPEN WITH SLIGHT LOADING. THE POSTED SPEED…

07V534000 · Engine And Engine Cooling:Engine:Diesel

Reported: 2007-11-20 (November 20, 2007)

ON CERTAIN TRUCKS EQUIPPED WITH CATERPILLAR C7 OR C9 DIESEL ENGINES, A FUEL PUMP FAILURE WILL DILUTE THE ENGINE OIL WITH FUEL AND MAY CAUSE THE TURBOCHARGER TO FAIL AND CAUSE LOSS OF THROTTLE CONTROL WITH WARNING.

Consequence: THE WARNING MAY BE EXPERIENCED BY ERRATIC PERFORMANCE AND EXCESSIVE SMOKE. BOTH OF THESE CONDITIONS MAY RESULT IN A SITUATION WHERE A VEHICLE CRASH COULD OCCUR.

Remedy: FORD IS WORKING WITH CATERPILLAR TO HAVE THE FUEL PUMP REPLACED FREE OF CHARGE (PLEASE SEE 07E053). OWNERS MAY CONTACT CATERPILLAR AT 309-675-6496 OR FORD AT 1-800-392-3673.

Reported: 2008-08-25 (August 25, 2008)

FORD IS RECALLING 2,811 MY 2008-2009 F-650 AND F-750 TRUCKS EQUIPPED WITH C7 OR C9 CATERPILLAR DIESEL ENGINES. THE FUEL PUMP OUTLET CHECK PLUG MAY BE CRACKED WHICH COULD CAUSE FUEL TO SPRAY AND LEAK.

Consequence: FUEL LEAKAGE, IN THE PRESENCE OF AN IGNITION SOURCE, COULD RESULT IN A FIRE.

Remedy: FORD IS WORKING WITH CATERPILLAR (PLEASE SEE 08E042). CATERPILLAR WILL REPLACE THE FUEL PUMP PLUG FREE OF CHARGE. OWNERS MAY CONTACT CATERPILLAR AT 1-800-447-4986, OR FORD AT 1-800-343-5338 (FLEET OWNERS) OR 1-866-906-9811 (MOTOR HOME OWNERS).

09V456000 · Equipment:Appliance:Air Conditioner

Reported: 2009-11-19 (November 19, 2009)

CUES, INC. IS RECALLING CERTAIN MODEL YEAR 2007-2009 SERVICE VEHICLES BUILT ON VARIOUS CHASSIS EQUIPPED WITH CARRIER AIRV CEILING UNITS, MODEL NOS. 68RV0010BA, 68RV0012CA, AND 68RV0010EA. THE HEATER ELEMENT IN THESE ASSEMBLIES MAY BE DEFECTIVE WHICH COULD CAUSE THE UNIT TO CREATE SMOKE OR SPARKS.

Consequence: THIS DEFECT COULD RESULT IN A FIRE.

Remedy: CUES IS WORKING WITH CARRIER TO REMEDY THE AFFECTED VEHICLES. REPAIRS WILL BE MADE BY CARRIER FREE OF CHARGE. PLEASE SEE CARRIER'S DEFECT REPORT 09E-053. THE SAFETY RECALL BEGAN ON JANUARY 5, 2010. OWNERS MAY CONTACT CARRIER AT 1-877-584-7059 OR CUES AT 1-407-849-0190.

10V398000 · Engine And Engine Cooling

Reported: 2010-09-09 (September 9, 2010)

FORD IS RECALLING CERTAIN MODEL YEAR 2008-2009 F-650 AND F-750 TRUCKS EQUIPPED WITH CATERPILLAR C7 ENGINE. THE #5 AND #6 FUEL LINES MAY CRACK AT THE FASTENING POINT TO THE COMMON FUEL RAIL AND CAUSE A FUEL LEAK.

Consequence: A FUEL LEAK, IN THE PRESENCE OF AN IGNITION SOURCE, CAN RESULT IN A VEHICLE FIRE.

Remedy: FORD IS WORKING WITH CATERPILLAR TO NOTIFY OWNERS AND REPAIRS WILL BE PERFORMED BY AUTHORIZED CATERPILLAR DEALERS FREE OF CHARGE. THE SAFETY RECALL BEGAN ON SEPTEMBER 8, 2010. OWNERS MAY CONTACT CATERPILLAR TOLL-FREE AT 1-800-447-4986 OR FORD AT 1-866-436-7332.
