MICHELIN TIRE FAILURE. CATASTROPHIC TIRE BLOWOUT-FRONT TIRE. FORD F350 TRUCK PULLING A 5TH WHEEL TRAILER. 3 VEHICLES INVOLVED ON CALIFORNIA INTERSTATE HIGHWAY 101. WHILE TOWING A 5TH WHEEL TRAILER I HAD A FRONT LEFT TIRE BLOW. THE TIRE BLEW DURING A RIGHT TURN/BEND IN THE HIGHWAY. I COULD ONLY MAINTAIN THE VEHICLE IN A STRAIGHT LINE UNTIL WE STOPPED. TL* THE CONTACT OWNS A 2011 FORD F-350 SUPER DUTY. WHILE DRIVING 55 MPH, THE VEHICLE STALLED AND AN UNKNOWN WARNING INDICATOR ILLUMINATED ON THE INSTRUMENT PANEL. THE VEHICLE WAS TOWED TO THE DEALER FOR DIAGNOSTIC TESTING, EGT SENOR #4 CIRCUIT OPEN, EGT SENSOR WAS REPLACED. THE VEHICLE WAS REPAIRED, BUT THE FAILURE RECURRED. THE MANUFACTURER WAS NOT NOTIFIED. THE FAILURE MILEAGE WAS 105,451.

WHILE USUALLY TRAVELING AT HIGHWAY SPEEDS AND HITTING SMALL BUMPS SUCH AS EXPANSION JOINTS ON BRIDGES THE TRUCK WILL SHAKE VIOLENTLY. IT HAS DONE THIS WITH VARYING DEGREES, AND MOST RECENTLY HAS GOTTEN WORSE. THE MOST EXTREME AND RECENT EPISODE WAS HAVING TO SLOW DOWN TO ABOUT 10 MPH ON THE INTERSTATE BEFORE IT STOPPED, AND IN PROCESS NEARLY BEING HIT FROM BEHIND BY A TRACTOR TRAILER. I WAS DRIVING EAST ON I-90 NEAR BOZEMAN, MT AT ABOUT 70MPH WHEN A WARNING APPEARED ON MY DASH TELLING ME TO PULL OVER TO A SAFE LOCATION IMMEDIATELY. THERE WAS AN OFF RAMP ABOUT 1/4 MILE AHEAD AND I BEGAN TO SLOW DOWN TO TAKE THE OFF RAMP WHEN THE ENGINE SHUT DOWN COMPLETELY AND LUCKILY, I WAS ABLE TO BRING IT TO A STOP BARELY ON THE OFF RAMP WITHIN ONLY A FEW FEET OF THE INTERSTATE HIGHWAY. PURCHASED 8/30/2011. 1ST SERVICE CALL NOV/2011: NOTICED AT HWY SPEEDS AND/OR ACCELERATING TO PASS OR ENTER HWY TRUCK WOULD MISS/CUT OUT. SEVERAL TIMES IT DIED AS TRUCK WAS ENTERING HWY. TRUCK WAS ALSO SHIFTING HARD WHEN GOING INTO DRIVE FROM PARK. DEALER STATED THEY COULDN'T DUPLICATE PROBLEM AT INITIAL VISIT. WHILE DRIVING AT HIGHWAY SPEED, THE VEHICLE, A 2011 FORD F-350 SUPER DUTY GAVE A WARNING TO PULL SAFELY OVER - WITH NO WARNING THE ENGINE STOPPED. THUS RENDERING THE STEERING AND THE BRAKING SIGNIFICANTLY DEGRADED. THANKFULLY, MY WIFE AND I WERE ABLE TO PULL INTO A NEARBY PARKING LOT AND WAIT FOR A TOW TO THE FORD DEALER. Reported: 2014-08-08 (August 8, 2014)
Vanair Manufacturing, Inc. (Vanair) is recalling certain model year 2009-2013 Ford F-350, F-450, and F-550 SuperDuty chassis modified by Vanair and equipped with certain Muncie Power Products PTO Assemblies used with model 60-85 UDSM, 125-185 USDM, and Genair 125-185 driven air compressor systems. In the affected vehicles, oil may leak from the PTO pressure switch onto a thermal blanket covering the exhaust.
Consequence: The leaking oil may soak into the thermal blanket covering the exhaust, which when combined with heat from exhaust, may result in an increased risk of fire.
Remedy: Vanair will notify owners, and dealers will inspect and install a new pressure switch kit that moves the switch away from the exhaust system, free of charge. The recall began in November 2014. Owners may contact Vanair customer service at 1-800-526-8817.

Versalift East, LLC (Versalift) is recalling certain Aerial devices manufactured on 2009-2013 Ford, F-350, F-450, and F-550 Super Duty 6.8L gas engine chassis, modified by Versalift and equipped with certain Muncie Power Products FR67 PTO Assemblies. In the affected vehicles, oil may leak from the PTO pressure switch onto a thermal blanket covering the exhaust.
Consequence: The leaking oil may soak into the thermal blanket covering the exhaust, which when combined with heat from exhaust, may result in an increased risk of fire.
Remedy: Versalift will notify owners, and dealers will install a new pressure switch kit that moves the switch away from the exhaust system, free of charge. The recall began on September 14, 2014. Owners may contact Versalift customer service at 1-888-866-1400.

Jerr-Dan Corporation (Jerr-Dan) is recalling certain model year 2009-2014 Ford F-350 Super Duty, F-450 Super Duty, and F-550 Superduty chassis modified by Jerr-Dan and equipped with certain Muncie Power Products PTO Assemblies. In the affected vehicles, oil may leak from the PTO pressure switch onto a thermal blanket covering the exhaust.
