Reported: 1991-01-06 (January 6, 1991)

PITMAN ARM PINCH BOLTS MAY HAVE BEEN INADEQUATELY TORQUED.

Consequence: THIS COULD ALLOW PITMAN ARM TO SLIP ON THE STEERING GEAROUTPUT SHAFT RESULTING IN LOSS OF CONTROL AND AN ACCIDENT.

Remedy: RETORQUE PINCH BOLTS AND RESET AS NECESSARY.

91V043000 · Service Brakes, Air

Reported: 1991-03-14 (March 14, 1991)

AIR BRAKE SYSTEM WAS MODIFIED BY ADDING A LONGER AIR LINE BETWEEN THE ABS SOLENOID AND THE TRANSFER TEE TO SIMPLIFY MANUFACTURING. THIS MODIFICATION CAUSED THE TRUCKS TO BE IN NON-COMPLIANCE WITH FMVSS 121, BRAKE TIMING TEST.

Consequence: THE MISTIMED BRAKE AND ABS SYSTEM MAY CAUSE THE TRUCK TOFAIL THE STOPPING DISTANCE REQUIREMENT OF THE FMVSS, WHICH MAY LEAD TO ANACCIDENT.

Remedy: REMOVE THE EXTRA LENGTH OF AIR LINE AND INSTALL THE TRANSFER TEE AT THE ABS SOLENOID.

91V066000 · Engine And Engine Cooling:Cooling System:Fan

Reported: 1991-04-18 (April 18, 1991)

THE ENGINE FAN MAY EXPERIENCE A RESONANT FREQUENCY CAUSING PREMATURE FAN FAILURE, AND SEPARATION FROM THE ENGINE.

Consequence: ENGINE FAN SEPARATION CAN CAUSE DEATH OR INJURY TO ANYPERSON IN THE PATH OF THE FAN.

Remedy: REPLACE ENGINE FAN WITH FAN FROM ANOTHER MANUFACTURER.
