TL* THE CONTACT OWNS A 1997 GMC SUBURBAN. THE CONTACT STATED THAT AFTER TURNING THE VEHICLE OFF SHE WAS UNABLE TO RESTART IT DUE TO A FAULTY IGNITION SWITCH. THE CONTACT INDICATED THAT AFTER INSERTING THE KEY SHE WAS UNABLE TO TURN THE IGNITION SWITCH TO START THE ENGINE. THE CONTACT INDICATED THAT THE IGNITION SWITCH WAS REPLACED MULTIPLE TIMES FOR THE SAME DEFECT. THE FAILURE WAS NOT DIAGNOSED OR REPAIRED. THE MANUFACTURER WAS NOT NOTIFIED OF THE FAILURE. THE FAILURE MILEAGE WAS 169,000 .

VEHICLE DESCRIPTION: GLAVAL CONVERSION VANS AND PICKUP TRUCKS. THE RETAINER CLIPS WERE NOT REMOVED FROM THE REAR BRAKE DRUM WHEEL STUD BOLTS PRIOR TO THE INSTALLATION OF A CUSTOM WHEEL.
Consequence: THE WHEEL CAN APPEAR TO BE OR CAN ACTUALLY GO OUT OF BALANCE AND COULD CRACK IN THE LUG NUT AREA INCREASING THE RISK OF A VEHICLE CRASH.
Remedy: DEALERS WILL INSPECT THE WHEELS AND REMOVE THE RETAINER CLIPS IF STILL IN PLACE AND ALSO INSPECT WHEELS FOR CRACKS (INCLUDING THE FRONT WHEELS, IF THE TIRES HAVE BEEN ROTATED). IF THE WHEELS ARE CRACKED IN THE LUG NUT AREA, THE WHEELS WILL BE REPLACED.
