TL* THE CONTACT OWNED A 2021 FOREST RIVER FLAGSTAFF MICROLIGHT. THE CONTACT STATED WHILE DRIVING AT 40 MPH, THE BRAKES FAILED TO ENGAGE CAUSING THE VEHICLE TO CRASH INTO ANOTHER VEHICLE. I TOOK POSSESSION OF MY BRAND NEW E-PRO COACH ON MARCH 12, 2021 AND ON MARCH 31,2021 THE AXLE DETACHED FROM THE FRAME AND CAUSE EXTENSIVE DAMAGE. LUCKILY I WAS TRAVEL AT APPROXIMATELY 5 MILES PER HOUR. I COULD HAVE BEEN SEVERELY INJURED AS WELL AS ANY PASSENGERS, OTHER TRAVELERS, BYSTANDERS ETC. IF THIS OCCURRED AT A HIGHER RATE OF SPEED OR IN ANY OTHER AREA. On the morning May 22nd, just 45 minutes after we left our home in Portland, our trailer burst into flames on 1-84 off exit 44 near Cascade Locks. It was inexplicable as to why this happened. It was a brand new trailer with less than 2k miles on it and purchased in December 2020. As you can see from the pictures, it was a total loss. We are filing this report to help protect others. The contact owns a 2021 Forest River Flagstaff Lite Weight Trailer towed by a 2015 GMC 2500. The contact stated while driving 65 MPH and towing the trailer, two full 7.5 propane tanks detached from the trailer and both were being dragged behind the RV. The contact was notified by another driver that the tanks were being dragged behind the RV. The contact pulled over safely and placed both propane tanks into the trailer. The contact owns a 2021 Forest River Flagstaff RV. The contact stated that the front driver’s side tire was significantly worn and damaged after driving approximately 200-500 miles. The vehicle was taken to the dealer, where it was diagnosed that a bearing retainer nut had detached and caused the tire damage. The dealer made an unknown adjustment; however, the failure recurred. Reported: 2021-04-05 (April 5, 2021)
Forest River, Inc. (Forest River) is recalling certain 2021 Forest River Rockwood and Flagstaff fifth wheels recreational vehicles. The pin box may be under-rated for the Gross Vehicle Weight Rating (GVWR) of the vehicle.
Consequence: The underrated pin-box may fail, increasing the risk of a crash.
Remedy: Forest River will notify owners, and dealers will inspect, and if necessary replace the pin box, free of charge. Owner letters were mailed May 7, 2021. Owners may contact Forest River customer service at 1-574-642-8943. Forest River's number for this recall is 10C-1319.

Forest River, Inc. (Forest River) is recalling certain 2021-2022 Coachmen Adrenaline, Apex, Catalina, Freedom Express, Spirit, East to West Alta, Della Terra, Silver Lake, Forest River Cedar Creek, Cherokee, Columbus, Flagstaff, Impression, Ozark, Rockwood, Sabre, Salem, Sandstorm, Shockwave, Stealth, Stealth Evo, Vengeance, Vibe, Wildwood, XLR Toyhauler, Palomino Puma, Prime Time Avenger, Crusader, Lacrosse, Sanibel, Shasta Oasis, 2022 Coachmen Cross Trail, Prism, Forest River Aurora, Rainier, Wildcat, Work and Play, Prime Time Tracer, 2021-2023 Coachmen Leprechaun, and 2022-2023 Coachmen Freelander fifth wheel and travel trailer recreational vehicles. The quick disconnect fittings in the LP gas system may be cracked, causing a gas leak.
Consequence: A gas leak in the presence of an ignition source can increase the risk of a fire.
Remedy: Dealers will replace the quick disconnect and perform a LP system check, free of charge. Owner notification letters were mailed May 20, 2022. Owners may contact the applicable customer service number for their vehicle model. Please see the recall report for the list of phone numbers.
