A 2001 EAST MANUFACTURING FRAMLESS DUMP TRUCK'S HYDRAULIC CYLINDER SYSTEM FAILED WHILE THE VEHICLE WAS UNLOADING GRAVEL ON A FLAT CONCRETE PAD. THE FAILURE OF THE CYLINDER OCCURRED AT THE FOURTH CYLINDER, IT BROKE INTO TWO PIECES, CAUSING THE TRAILER TO FAIL ONTO THE TRUCK'S CAB. THE CYLINDER WAS MANUFACTURED BY THE PARKER HANNIFIN CORP. THE INCIDENT CAUSED INJURY TO THE DRIVER AND PROPERTY DAMAGE.

CERTAIN FLATBED, DUMP, TRUCK DOLLY, UNLOADER, FRAMELESS, AND PUP TRAILERS FAIL TO COMPLY WITH THE REQUIREMENTS OF FEDERAL MOTOR VEHICLE SAFETY STANDARD NO. 121, "AIR BRAKE SYSTEMS." THESE VEHICLES HAVE INACCURATE WEIGHT-RELATED INFORMATION ON THE CERTIFICATION LABELS.
Consequence: IF THAT INACCURATE INFORMATION IS RELIED UPON BY THE OWNER OF THE VEHICLE, THEN EXTRA WEIGHT LOADED ONTO A VEHICLE COULD AFFECT THE AIR BRAKE SYSTEM PERFORMANCE.
Remedy: DEALERS WILL CORRECT THE CERTIFICATION LABEL. OWNER NOTIFICATION IS EXPECTED TO BEGIN DURING SEPTEMBER OR OCTOBER 2002. OWNERS WHO TAKE THEIR VEHICLES TO AN AUTHORIZED DEALER ON AN AGREED UPON SERVICE DATE AND DO NOT RECEIVE THE FREE REMEDY WITHIN A REASONABLE TIME SHOULD CONTACT EAST MANUFACTURING AT 1-330-325-9921.
