TL* THE CONTACT OWNS A 2015 TOYOTA YARIS. THE CONTACT STATED THAT AFTER REVERSING OUT OF PARKING, THE VEHICLE WAS SHIFTED TO DRIVE AND THE VEHICLE ROLLED BACKWARDS WITHOUT WARNING. THE CONTACT DEPRESSED THE BRAKE PEDAL BUT WAS UNABLE TO STOP THE VEHICLE. THE VEHICLE CRASHED INTO THE REAR OF ANOTHER VEHICLE. THERE WERE NO INJURIES REPORTED AND NO POLICE REPORT WAS FILED. THE VEHICLE WAS NOT DIAGNOSED OR REPAIRED. THE MANUFACTURER WAS NOT NOTIFIED OF THE FAILURE. THE FAILURE MILEAGE WAS 100. THE VIN WAS NOT PROVIDED.

THE TOYOTA 4 DOOR YARIS SUDDENLY ACCELERATED UPON INITIATING MOVEMENT FROM A RIGHT TURN STOP. THE VEHICLE ACCELERATED TO APPROXIMATELY 30 MPH WITHIN 100 FT. AND CAUSED A HEAD ON WITH FORD 250 TRUCK. UPON IMPACT, THE STEERING WHEEL AIR BAG DEPLOYED AND DRIVER OF THE YARIS SUSTAINED A CONCUSSION, FRACTURED STERNUM AND SEAT BELT BRUISING. WHILE I WAS PARKING MY CAR IN MY DRIVE WAY AND I TRIED TO STOP WITH THE BREAKS . I DIDN'T STOP EVEN THOUGH I WAS BREAKING HARD AND THE SPEED EXCELLERATED AND I HAD TROUBLE CONTROLING THE STEERING WHEEL , ITS MOVED IT SELF.
