TL* THE CONTACT OWNS A 2010 TOYOTA LAND CRUISER. WHILE DRIVING OUT OF A PARKING LOT, THE FOOT REST TO THE LEFT OF THE BRAKE PEDAL DETACHED. AN INDEPENDENT MECHANIC REPLACED THE FOOT REST; HOWEVER, THE FAILURE RECURRED. THE VEHICLE WAS TAKEN TO A DEALER, BUT WAS NOT DIAGNOSED OR REPAIRED. THE MANUFACTURER WAS NOT NOTIFIED OF THE FAILURE. THE FAILURE MILEAGE WAS 89,804. THE VIN WAS NOT AVAILABLE.

TL* THE CONTACT OWNS A 2010 TOYOTA LAND CRUISER. WHEN THE PUSH TO START BUTTON WAS APPLIED, AN ABNORMAL POPPING NOISE WAS HEARD. THE VEHICLE WAS ENGAGED IN THE PARK POSITION AT THE TIME OF THE FAILURE. ALSO, WHEN THE BRAKE PEDAL WAS APPLIED, THE VEHICLE FAILED TO RESPOND. AS A RESULT, THE VEHICLE CRASHED INTO A FORD FOCUS AND TWO OTHER VEHICLES. ALSO, SMOKE EMERGED FROM UNDERNEATH THE VEHICLE. THERE WERE NO WARNING INDICATORS ILLUMINATED. THE AIR BAGS DID NOT DEPLOY.
