ON JANUARY 10, 2020 APPROXIMATELY 09:30 AM, WHEN IN A PARKING GARAGE I STOPPED THE CAR AND WENT TO PUT IT INTO REVERSE TO BACK INTO A STALL WHEN THE CAR ACCELERATED FORWARD IMPACTING A PILLAR WHEN I LOOKED DOWN THE CAR WAS IN NEUTRAL AND THE HAZARD LIGHTS WERE ON. TL* THE CONTACT OWNS A 2018 TESLA MODEL S. WHILE DRIVING AT 5 MPH THE VEHICLE INDEPENDENTLY ACCELERATED, JERKED AND STALLED. THE SEAT BELT WARNING LIGHTS WERE ILLUMINATED. ON NOVEMBER 14, 2019 MY CAR WAS INVOLVED IN AN ACCIDENT THAT I STRONGLY BELIEVE THERE WAS A MALFUNCTION WITH THE ACCELERATOR SENSORS OR THE ACCELERATOR ITSELF. THE VEHICLE SUDDENLY ACCELERATED AGGRESSIVELY AS MY SISTER WAS PARKING THE CAR. SHE WAS GOING SLOW AS SHE PARKED THE VEHICLE AND AS SHE DROVE INTO THE STALL, THE VEHICLE SPED UP AGGRESSIVELY AND SHE COULD NOT STOP IT. The contact owns a 2018 Tesla Model 3. The contact stated that the vehicle had begun swaying while driving approximately 60 MPH. Additionally, the contact noticed a creaking noise coming from underneath the vehicle. The contact pulled into a parking lot and the vehicle suddenly stalled. The contact noticed that the control arm had detached. The vehicle was towed to the dealer but was not yet diagnosed. The vehicle was not repaired. The manufacturer and dealer were informed of the failure. The approximate failure mileage was 33,000.

1. The Power Conversion System (PCS), still in the vehicle and not available for inspection 2. 2 of the 3 controllers have failed, resulting in limited AC charging (16A out of 48A max). The PCS helps regulate AC and DC electricity and connects the 12V batter to the HV battery. If it fails, it could lead to dangerous situations since the electricity from the HV battery wouldn't be as controlled.
