DT*: THE CONTACT STATED WHILE DRIVING A COMPANY GARBAGE TRUCK AT 45MPH ANOTHER VEHICLE PULLED OUT IN FRONT OF THE TRUCK. AFTER SWERVING TO AVOID A COLLISION THE VEHICLE CRASHED INTO A SEMI-TRUCK. THE DRIVER WAS THROWN FROM THE VEHICLE AND WAS PRONOUNCED DECEASED AT THE SCENE. THE CONTACT'S SON WAS DRIVING THE GARBAGE TRUCK. THERE WAS A POLICE REPORT FILED AND THE FIRE DEPARTMENT WAS ALERTED AS A PRECAUTION. TWO HOURS BEFORE THE CRASH THE CONTACT WAS INFORMED THE BRAKES WERE MALFUNCTIONING.
