THIS IS A 2008 CHEVROLET 3500 CUTAWAY SCHOOL BUS WITH 24,926 MILES ON. THE DRIVER WAS COMING TO A STOP TRAVELING APPROXIMATELY 15 MPH. HE APPLIED THE BRAKES AND THE PEDAL WENT TO THE FLOOR. THE BUS THEN REAR ENDED A ANOTHER VAN WHICH THEN HIT THE REAR OF A POSTAL TRUCK. THE DRIVERS OF THE TWO OTHER VEHICLES WERE TAKEN AWAY BY AMBULANCE COMPLAINING OF BACK INJURIES. VAN HAS ALWAYS BEEN USED FOR RESIDENTIAL ELECTRICAL WORK. NOTHING HAS EVER BEEN HUNG ON THE SIDE DOORS, HOWEVER, THE HINGES ARE SAGGING TO THE POINT THAT THE DOORS ARE OVERLAPPING AND ARE NEARLY UNUSABLE. WE KNOW OF ANOTHER CHEVY EXPRESS VAN THAT IS DOING THE SAME THING. *TR

While driving at highway speed, the factory-installed serpentine belt on my 2023 Chevrolet Express 3500 (6.6L V8, approximately 24,000 miles, purchased new, factory-maintained per schedule) failed suddenly and without warning. The belt on this engine drives the water pump and other critical accessories, so the failure caused immediate loss of engine cooling and rapid, severe overheating while the vehicle was in motion in traffic. Vehicle is upfitted as a school bus. Forward fuel tank strap failed in operation. Our company operates more than 100 of these vehicles and we experience a failure of fuel tank straps at a rate of about one vehicle per month. If the 2nd strap would also fail the gasoline tank would become dislodged and present a fire hazard. This is even more critical as these vehicles are carrying students as a school bus.
